Adult Community Case Managers at LEAP coordinate supports and services that promote independence, inclusion, and quality of life. They work closely with the supported person, their natural support (family, friends, etc.) and their service providers to develop and monitor a Person-Centered Plan that meets the individual’s unique needs and goals. This role includes regular visits with the individual at their home or in their community.
